60 degrees for offhand flourishing?

Apologies if this is something that everyone already knows but when we refer to holding the pen at  60 degrees for doing flourishing (as mentioned in Ornate Pictorial Calligraphy and Hoang’s videos),are we talking about pen being elevated 60 degrees from paper or from where?  Im trying to do the exercises in the book but the shading is hard when they run parallel to the lower border of the paper (so shading strokes but not down strokes).  Is there a way to resolve this?
