I can easily bring the flange out away from the holder by increasing the length of the flange itself. Most modern store bought holders are set for a standard nib (Leonardt Principal, 303, 404, Nikko G, etc)
Also the diameter of the flange has a direct result on the nib placement and angle. Dr. Joe Vitolo did an amazing pdf (after some cheesy photos I sent him of the effect) regarding this in an email discussion I had with him and Nick D'Aquanno. I will see if I can get his permission to repost it here.
You can have the nib before or past the center axis of the shaft. There are penman in the past and now who prefer it not centered. By having the nib centered it just allows the nib to remain on axis if you manipulate the holder rotation when you write. Even though the old timers stated a nib must be centered, there is no constant when you create art (and calligraphy definitely an art) in my opinion.
If people don't break rules we wouldn't have the Picaso's, Jimi Hendrix's, Nikola Tesla and Elon Musk's in the world. All were considered to be nutcases and doing it all wrong at one point in their lives.
